29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed  JOB SITE: 130 Grimsley Station Road, Saint Louis, MO 63129, on or about 7/24/2017, workers were observed using a Werner Aluminum extension ladder, and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed.  The workers were exposed to a fall hazard.

29 CFR 1926.102(a)(1): The employer did not ensure that each affected employee used appropriate eye or face protection when exposed to eye or face hazards from flying particles:  JOB SITE: 130 Grimsley Station Road, Saint Louis, MO 63129, on or about 7/24/2017, workers were observed on top of the roof, actively installing new roof shingles, operating pneumatic nail gun without any form of eye or face protection. The workers were exposed to struck-by hazard.  ABOVE THE REST ROOFING & EXTERIORS was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.102(a)(1), which was contained in OSHA inspection number 962877, citation number 2, item number 1, issued on 3/19/2014 and was affirmed as a final order on 5/30/2014, with respect to a workplace located at 423 W. Woodbine Avenue, Kirkwood, MO 63122.  ABOVE THE REST ROOFING & EXTERIORS was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.102(a)(1), which was contained in OSHA inspection number 428211, citation number 1, item number 1, issued on 8/23/2012 and was affirmed as a final order on 11/21/2013, with respect to a workplace located at 3596 Triple Crown Drive, Florissant, MO 63034.
