29 CFR 1926.416(a)(1): Employees were permitted to work in proximity to electric power circuits and were not protected against electric shock by de-energizing and grounding the circuits or effectively guarding the circuits by insulation or other means:  JOB SITE: 423 W. Woodbine Avenue, Kirkwood, MO 63122, on or about 3/11/2014 a worker was observed actively installing new roof shingles in close proximity to electric power circuits and was not protected against electric shock or effectively guarding the circuits by insulation or other mean.  The worker was exposed to electrical hazard.

29 CFR 1926.502(d)(15): Anchorages used for attachment of personal fall arrest equipment were not capable of supporting at least 5,000 pounds per employee attached:  JOB SITE: 423 W. Woodbine Avenue, Kirkwood, MO 63122, on or about 3/11/2014 at this work site, observed an anchor plate over the rig of the roof, anchored & secured on the west side of the roof with the other portion of the anchor plate on the east side of the roof was left unsecured and the workers had fastened three (3) independent lifelines on the ring of the anchor plate.  The workers were exposed to a fall hazard.

Woodbine Avenue, Kirkwood, MO 63122 on or about 3/11/2014 at this work site, a worker was observed climbing up a Werner aluminum extension ladder with a stack of roofing shingles and was not grasping the ladder with at least one hand while the worker was progressing up the ladder.  The worker was exposed to a fall hazard.

Woodbine Avenue, Kirkwood, MO 63122, on or about 3/11/2014, at this work site workers were observed on top of a residential roof, operating pneumatic nails guns, actively installing new roof shingles without any form of eye or face protection.  The workers were exposed to a struck-by hazard.  The ABOVE THE REST ROOFING & EXTERIORS was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.102(a)(1), which was contained in OSHA inspection number 428211, citation number 1, item number 1, issued on 8/23/2012, and was affirmed as a final order on 9/25/2012, with respect to a workplace located 3956 Triple Crown Drive, Florissant, MO 63034.  The ABOVE THE REST ROOFING & EXTERIORS was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.102(a)(1), which was contained in OSHA inspection number 315979708, citation number 1, item number 2, issued on 12/05/2011, and was affirmed as a final order on 1/2/2012, with respect to a workplace located 11824 Beaverton Drive, Bridgeton, MO 63044.
