WAS PREVIOUSLY CITED FOR VIOLATION OF THIS OCCUPATIONAL SAFETY AND HEALTH STANDARD OR ITS EQUIVALENT STANDARD 1910.305(g)(2)(iii), WHICH WAS CONTAINED IN OSHA INPSECTION NUMBER 314058884, CITATION 002, ITEM NUMBER 001 AND WAS AFFIRMED AS A FINAL ORDER ON 07/07/2011, WITH RESPECT TO A WORKPLACE LOCATED AT 8002 N. 288 STREET, VALLEY, NEBRASKA.
