He was not working from a scaffold, sloping ladder and was not protected by a safety belt and lifelines meeting the requirements of 1915.159 and 1915.160

At the upper level the forward exit access was blocked by ships equipment and five fifty five gallon drums of oil. b) Onboard USCG vessel WALNUT A easy and open access to each exit was not maintained for employees working at the lower level of the cargo hold. The aft passagewayt from the upper level at 2-45-3 was blocked by tank top guard.
