29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet or more above lower levels was not protected by guardrail systems, safety net systems, or personal fall arrest systems: At the job site, four (4) workers were observed installing asphalt shingles on a roof with a slope of approximately 8/12. One employee was wearing improper fall protection (body belt) and the other three workers were not using any type of fall protection. The workers were exposed to falls between approximately 10 feet, 5 inches and 21 feet, 6 inches. Repeat Information: On Top Contracting, LLC. was previously cited for a violation of this occupational safety and health standard or its equivalent standard of 29 CFR 1926.501(B)(13), which was contained in OSHA inspection number 313111577, citation number 1, item 6, issued on 04/30/2009, and was affirmed as a final order on 05/19/2009, with respect to a workplace located at 305 S. 29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access: At this job site, workers were observed using an extension ladder to gain access on and off the residential roof, approximately 10 feet, 6 inches eave to ground level, where the ladder side rails did not extend at least 3 feet above the upper landing surface. Workers were exposed to a potential fall hazard.
