On 3/14/2014, four employees from On Top Contracting, LLC were working on the roof of a two-story residential building located at 1113 Duxbury Lane, OFallon MO. Three employees were observed not wearing eye protection while using pneumatic nailers. On Top Contracting, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.102(a)(1), which was contained in OSHA inspection number 315785501, citation number 2, item number 1, and was affirmed as a final order on 8/8/2012, with respect to a workplace located at 11737 Beaverton Drive, Bridgeton , MO 63044.

Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net system, or personal fall arrest system. On 3/14/2014, four employees from On Top Contracting, LLC were working on the roof of a two-story residential building located at 1113 Duxbury Lane, OFallon MO. They were not utilizing guardrail systems, a safety net system, or a personal fall arrest system. The employees were observed on the roof of the building without fall protection, exposing them to a fall to the ground below. On Top Contracting, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 315785501, citation number 2, item number 2, and was affirmed as a final order on 8/8/2012, with respect to a workplace located at 11737 Beaverton Drive, Bridgeton , MO 63044.
