Pro Roofing & Associates, Incorporated was previously cited for a violation of this occupational safety and health standard or its equivalent standard (29 CFR 1926.102(a)(1) which was contained in OSHA inspection number 311822530, citation 01, item number 002, issued on August 10, 2009, with respect to a workplace at 2971 Waller Street, Jacksonville, Florida. The final order date was August 31, 2009.    29 CFR 1926.102(a)(1): Employees shall be provided with eye and face protection equipment when machines or operations present potential eye or face injury from physical, chemical, or radiation agents.    a. On or about June 5, 2012, an employee using a Hitachi NV83A2 pneumatic air nail gun did not have on safety glasses, exposing himself to being hit by flying debris or nails.

Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels shall be protected by guardrail systems, safety net system, or personal fall arrest system unless another provision in paragraph (b) of this section provides for an alternative fall protection measure.     a. On or about June 5, 2012, on a 3.5 pitch roof, three employees performing roofing operations such as applying Tomka felt and Weatherwood shingles were not protected from a 17 feet fall hazard by the use of a fall protection system.

29 CFR 1926.1053(b)(1): When portable ladders are used for access to an upper landing surface, the ladder side rails shall extend at least 3 feet (.9 m) above the upper landing surface to which the ladder is used to gain access.     a. On or about June 5, 2012, the 32 foot Werner extension ladder used to access the roof did not extend 3 feet above the eave, exposing employees to a 17 feet fall hazard.
