29 CFR 1926.100(a):     Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:    a) On the roof near the crematory stack, on or about June 5, 2012, an employee who was not wearing a hard hat was working around the load and hook while a crane was moving materials, resulting in a struck-by hazard.

29 CFR 1926.501(b)(11):     Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems:    a) On the roof near the crematory stack, on or about June 5, 2012, an employee who was working while standing on the 6:12 sloped roof was not utilizing any means of fall protection, resulting in a twelve-foot fall hazard.    Repeat Note:  Matthews International Corporation was previously cited for a violation of this occupational safety and health standard or its equivalent standard, 29 CFR 1926.501(b)(1), which was contained in OSHA inspection number 310800057, citation number 1, item number 1a, and was affirmed as a final order on October 21, 2008, with respect to a workplace located at 7519 North Allen Road, Peoria, IL 61614.

29 CFR 1926.503(a)(1):     The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:    a) On the roof near the crematory stack, on or about June 5, 2102, the employer did not have an adequate fall protection training program for an employee who was working while standing on the 6:12 sloped roof without fall protection, resulting in a twelve-foot fall hazard.    Repeat Note:  Matthews International Corporation was previously cited for a violation of this occupational safety and health standard or its equivalent standard, 29 CFR 1926.503(a)(1), which was contained in OSHA inspection number 310800057, citation number 1, item number 1a, and was affirmed as a final order on October 21, 2008, with respect to a workplace located at 7519 North Allen Road, Peoria, IL 61614.
