29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems: a) On the roof near the crematory stack, on or about June 5, 2012, an employee who was working while standing on the 6:12 sloped roof was not utilizing any means of fall protection, resulting in a twelve-foot fall hazard. Repeat Note: Matthews International Corporation was previously cited for a violation of this occupational safety and health standard or its equivalent standard, 29 CFR 1926.501(b)(1), which was contained in OSHA inspection number 310800057, citation number 1, item number 1a, and was affirmed as a final order on October 21, 2008, with respect to a workplace located at 7519 North Allen Road, Peoria, IL 61614.
