29 CFR 1926.95(a): Protective equipment, including personal protective equipment for eyes, face, head, and extremities, protective clothing, respiratory devices, and protective shields and barriers, was not provided, used, and maintained in a sanitary and reliable condition wherever it was necessary by reason of hazards of processes or environment, chemical hazards, radiological hazards, or mechanical irritants encountered in a manner capable of causing injury or impairment in the function of any part of the body through absorption, inhalation or physical contact:    a)  On June 6, 2012, or at times prior thereto, the controlling and exposing employer Bascon Corporation, failed to ensure the use of appropriate gloves while mixing and applying Portland cement.  The Portland cement was mixed with water in a wheelbarrow and applied onto CMU blocks.  The exposed employee was observed handling/applying Portland cement with his bare hands.     b) On June 6, 2012, or at times prior thereto, eye protection was not worn while mixing and applying Portland cement.  The Portland cement was mixed with water in a wheelbarrow and applied as a finish coat on the exterior walls.

The sander was used to sand wooden door jambs.
