a) Frontier Restoration LLC, 16750 Red Lane Street, Hudson, CO: Prior to the start of work, the controlling employer in charge of the worksite did not ensure a competent person was designated to conduct frequent inspections to identify and correct hazardous conditions capable of causing injury. On or about June 8, 2012, employees replacing an existing shingled 5/12 pitched roof were not using adequate fall protection. Condition exposed employees to a fall hazard of approximately 15 feet. OSHA defines a "competent person" as someone capable of identifying existing and predictable hazards in the surroundings or working conditions which are unsanitary, hazardous, or dangerous to employees, and who has authorization to take prompt corrective measures to eliminate them. Abatement Note: Abatement certification is required for this item (See enclosed Abatement-Certification Letter).

29 CFR 1926.501(b)(13): Employee(s) engaged in residential construction activities 6 feet or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest systems: a) Frontier Restoration LLC, 16750 Red Lane Street, Hudson, CO: Prior to the start of work, the controlling employer in charge of the worksite did not ensure the use of appropriate fall protection. On or about June 8, 2012, employees replacing an existing 5/12 pitched shingled roof were not using fall protection. Condition exposed employees to a fall hazard of approximately 15 feet. Frontier Restoration was previously cited for a violation of this Occupational Safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13) which was contained in OSHA inspection number 312143415, citation number 1, item number 2, issued on 09/04/2009, with respect to a workplace located at 11880 W. Independence Avenue, Lakewood, CO. Abatement Note: Abatement certification and documentation is required for this item (See enclosed Abatement-Certification Letter).
