Independence Avenue, Lakewood, CO 80226.  This citation became final on September 16, 2009.    Frontier Restoration LLC was previously cited for a violation of this Occupational Safety and Health Standard 29 CFR 1926.501(b)(13) or its equivalent which was contained in OSHA inspection 312142409, Citation 1, Item 3, issued September 3, 2009, with respect to a workplace located at 11647 Laurel Lane, Parker, CO 80134.  This citation became final on September 16, 2009.
