29 CFR 1926.252(a): An enclosed chute of wood, or equivalent material, was not used where materials were dropped more than 20 feet to points lying outside the exterior walls of the building(s): a) On or about May 28, 2013, Frontier Restoration LLC, as a controlling employer, did not ensure that J&PC Roofing (subcontractor) protected their employees from falling debris by ensuring that a chute was used to lower loose trash from a 34 feet roof to a dumpster on the ground.

29 CFR 1926.501(b)(13): Employee(s) engaged in residential construction activities 6 feet (1.8m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were other provisions of paragraph (b) of this section used to provide an alternative fall protection measure: a) On or about May 28, 2013, Frontier Restoration LLC, as a controlling employer, did not ensure that J&PC Roofing (subcontractor) protected their employees from falls greater than 6 feet while performing roofing work without adequate fall protection measures in place. Frontier Restoration LLC was previously cited for a violation of this Occupational Safety and Health Standard 29 CFR 1926.501(b)(13) or its equivalent which was contained in OSHA inspection 312143415, Citation 1, Item 2, issued August 21, 2009, with respect to a workplace located at 11880 W. Independence Avenue, Lakewood, CO 80226. This citation became final on September 16, 2009. Frontier Restoration LLC was previously cited for a violation of this Occupational Safety and Health Standard 29 CFR 1926.501(b)(13) or its equivalent which was contained in OSHA inspection 312142409, Citation 1, Item 3, issued September 3, 2009, with respect to a workplace located at 11647 Laurel Lane, Parker, CO 80134. This citation became final on September 16, 2009.
