29 CFR 1926.1051(a): Stairway(s) or ladder(s) were not provided at all personnel points of access where there was a break in elevation of 19 inches (48 cm) or more, or no ramp, runway, sloped embankment, or personnel hoist was provided: a) On or about May 17, 2013, Frontier Restoration LLC, as a controlling employer, did not ensure that J&PC Roofing (subcontractor) protected their roofing employees by providing a ladder or other adequate means of access to garage roofs. To reach garage roofs which were approximately 8 feet above the ground, employees were observed climbing the walls around garbage dumpsters and stepping up and across to the roofs.

29 CFR 1926.501(b)(13): Employee(s) engaged in residential construction activities 6 feet (1.8m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were other provisions of paragraph (b) of this section used to provide an alternative fall protection measure: a) On or about May 17, 2013, Frontier Restoration LLC, as a controlling employer, did not ensure that J&PC Roofing (subcontractor) protected their employees from falls greater than 6 feet while performing roofing work without adequate fall protection measures in place. Frontier Restoration LLC was previously cited for a violation of this Occupational Safety and Health Standard 29 CFR 1926.501(b)(13) or its equivalent which was contained in OSHA inspection 312143415, Citation 1, Item 2, issued August 21, 2009, with respect to a workplace located at 11880 W. Independence Avenue, Lakewood, CO 80226. This citation became final on September 16, 2009. Frontier Restoration LLC was previously cited for a violation of this Occupational Safety and Health Standard 29 CFR 1926.501(b)(13) or its equivalent which was contained in OSHA inspection 312142409, Citation 1, Item 3, issued September 3, 2009, with respect to a workplace located at 11647 Laurel Lane, Parker, CO 80134. This citation became final on September 16, 2009.
