Employees were exposed to the hazard of being caught in auger.

29 CFR 1910.27(b)(1)(v): The rungs of individual-rung ladders were not so designed that the foot cannot slide off the end. On or about June 7, 2012, and times prior thereto, the rungs of the individual-rung ladder in the man-lift shaft were not designed to prevent employees' feet from sliding off the ends of the rungs. The ladder is one of two ladders used for emergency escape from the gallery. Employees are exposed to the hazard of falls.
