29 CFR 1926.451(a)(1): Scaffold(s) and scaffold(s) component(s) were not capable of supporting, without failure, its own weight and at least four times the maximum intended load applied or transmitted to it:    a) On or about June 9, 2012 located at the Columbia Parkway Overpass project employees performed non-adjustable multipoint suspension scaffold containment floor/fence associated erection operations on which the outriggers/spacers were under-strength as fabricated, constructed and/or loaded as a system of temporary support and/or final design by the way the system and components were being erected and/or utilized, in that the scaffold component system was not capable of supporting at least four times the load applied as applied or transmitted.

29 CFR 1926.452(i)(8):  Scaffolds and scaffold components shall be designed by a registered professional engineer and shall be constructed and loaded in accordance with such design.     a) On or about June 9,2012 located at the Columbia Parkway overpass project employees performed non-adjustable multipoint suspension scaffold containment floor/fence associated  erection operations on which the outrigger/spacer components were not fabricated, constructed and/or loaded in accordance with registered professional engineer design specifications and/or equivalent redesign in that, outrigger/spacers as utilized were incapable of supporting required/ reasonably anticipated loads as a system of temporary support and/or final design.
