29 CFR 1926.405(g)(1)(ii): Attachment plugs for cords. When used as permitted in paragraphs (g)(1)(i)(C), (g)(1)(i)(F), or (g)(1)(i)(H) of this section, flexible cords were not equipped with an attachment plug and were not energized from a receptacle outlet: (a) Inside the control room for the lift station on or about June 14, 2012, and at times prior thereto, an extension cord did not have a plug and was not energized from an outlet as the cord was directly connected to a 110 VAC circuit breaker and neutral bar in an electrical panel.

29 CFR 1926.451(g)(1): Each employee on a scaffold more than 10 feet above a lower level was not protected from falling to that lower level: (a) Inside the lift station on or about June 14, 2012, employees on a tubular welded frame scaffold were not protected from falling to the bottoms of the concrete lift station. The scaffold's platform was 12 feet above the bottom of the lift station. Safety harnesses and lifelines were onsite, but they were not used.

29 CFR 1926.1053(b)(1): When portable ladders were used for access to an upper landing surface, the ladder side rails shall extend at least 3 feet (.9 m) above the upper landing surface to which the ladder is used to gain access; or, when such an extension is not possible because of the ladder's length, then the ladder was not secured at its top to a rigid support that will not deflect, and a grasping device, such as a grabrail, was not provided to assist employees in mounting and dismounting the ladder: (a) Inside the lift station on or about June 14, 2012, and at times prior thereto, the extension ladder did not extend three feet above the top of the lift station and there was no grabrail at the top of the station. It was 17.5 feet from the top to the bottom of the lift station.
