29 CFR 1926.50(c): A person who has a valid certificate in first-aid training was not available at the worksite to render first-aid: At the project site, on or about June 14, 2012 and times prior thereto, there was no person on-site who had a valid certificate in first-aid training from the U.S. Bureau of Mines, the American Red Cross, or equivalent training. Documentary evidence of certification was not be available at the worksite. NOTE: Distance from the worksite to the Guam Memorial Hospital is approximately 7 miles. Estimated time is about 17 minutes.

29 CFR 1926.404(b)(1)(i): Employer did not use either ground fault circuit interrupters as specified in paragraph (b)(1)(ii) of this section, or an assured equipment grounding conductor program as specified in paragraph (b)(1)(iii) of this section to protect employees on construction sites. At the south end of the building, on or about June 14, 2012 and times prior thereto, there was no ground-fault circuit-interrupter protection or an assured equipment grounding conductor program when extension cords were used to power a 110 VAC Makita HP2050 drill. The drill was used to secure junction boxes onto the concrete ceiling.

29 CFR 1926.405(g)(2)(iv): Flexible cords were not connected to devices and fittings so that strain relief is provided to prevent pull from being directly transmitted to joints or terminal screws: At the south end of the building, on or about June 14, 2012 and times prior thereto, there was no strain relief for a 50 foot extension cord used to provide temporary power to a Makita HP2050 drill. The outside insulation of the extension cord was pulled away from the plug. An employee used the drill to secure junction boxes onto the concrete ceiling.
