Located at Scolding Locks, 1520 W. Rogers Ave. Appleton, WI 54914:  On or about June 15, 2012, the employer did not record the following workplace injuries or illnesses on the OSHA Form 300 or equivalent for the calendar year 2011 and 2012.    On or about January 23, 2012, an employee was shocked with 240 volts on machine #14 in his right forearm.  The employee went to the hospital where he was treated with intravenous fluids and had an electrocardiogram performed.    Abatement Certification and Documentation is required
