29 CFR 1926.25(a): During the course of construction, alteration, or repairs, debris was not kept cleared from work areas, passageways, and stairs, in and around buildings or other structures: At the project site, on or about June 18, 2012, and times prior thereto, lumber with protruding nails and other debris lay within two feet of a pathway to the rear work area. Employees were exposed to trip and impalement hazards.

29 CFR 1926.404(b)(1)(i): Employer did not use either ground fault circuit interrupters as specified in paragraph (b)(1)(ii) of this section, or an assured equipment grounding conductor program as specified in paragraph (b)(1)(iii) of this section to protect employees on construction sites. At the project site on or about June 18, 2012, and at times prior thereto, there was no assured equipment grounding conductor program or ground-fault circuit-interrupter program when a 50 foot extension cord was used conjunction with a DeWalt DW511 drill. The 50 foot extension cord had an "Open Ground, thus not allowing the GFCI receptacle to trip. Employee was exposed to electrical shock and/or electrocution.
