29 CFR 1910.36(d)(2):  Fire doors were not maintained in proper operating condition:    (a) Dura-Bond Pipe, LLC, 2716 South Front Street, Steelton, PA - Employees using the outside exit door in the Powder room were exposed to an egress hazard when the door was jammed and in disrepair, on or about June 15, 2012.    Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

29 CFR 1910.37(a)(3):  Exits must be free and unobstructed:    (a)  Dura-Bond Pipe, LLC, 2716 South Front Street, Steelton, PA - Employees were exposed to an egress/fire hazard when the exit doors in the Powder room were blocked by equipment and bags of epoxy coating powder, on or about June 15, 2012.    (b)  Dura-Bond Pipe, LLC, 2716 South Front Street, Steelton, PA - Two (2) employees, working in the center of the OD area where pipes move on the line, were exposed to an egress/fire hazard because they had to climb over two (2) sets of I-beams which obstructed their ability to exit the area, on or about June 15, 2012.    Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

29 CFR 1910.134(g)(1)(i)(A):  The employer shall not permit respirators with tight-fitting facepieces to be worn by employees who have any facial hair that comes between the sealing surface of the facepiece and the face or that interferes with valve function:    (a) Dura-Bond Pipe, LLC, 2716 South Front Street, Steelton, PA - An employee working in the Powder room had a full beard while wearing a 3M Model 2091, dual-cartridge, half-mask full-faced respirator exposing the employee to toxic dusts, on or about June 15, 2012. DURA-BOND COATING, INC. WAS PREVIOUSLY CITED FOR A VIOLATION OF THIS OCCUPATIONAL SAFETY AND HEALTH ADMINISTRATION STANDARD OR ITS EQUIVALENT STANDARD 29 CFR 1910.178(p)(1) WHICH WAS CONTAINED IN OSHA INSPECTION NUMBER 312636806, CITATION NUMBER 1, ITEM NUMBER 3, AND WAS AFFIRMED AS A FINAL ORDER ON 5/4/10 WITH RESPECT TO A WORKPLACE LOCATED AT MCKEESPORT, PA.    Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.
