29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:    (a) South AC - The roller conveyor line, used to move pipes out of the building, either upriver to the yard or down river to coating, did not have machine guarding to protect employees who had to walk between the stands of conveyor rollers to move from the control station side of the conveyor line to the mill side of the conveyor line to load pipes from the mill onto the roller conveyor line, exposing employees to struck-by and caught between hazards.  On or about October 12, 2016, while moving to the mill side of the roller conveyor line, an employee on the down river end of South AC got struck in the back of the leg by a piece of steel pipe moving in the upriver direction on the roller conveyor line, and this piece of pipe pushed the employee's leg into an idle roller, causing crushing injuries resulting in amputation of the lower part of the employee's right leg.    Abatement certification and documentation required within 10 days after abatement date. The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement. Abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photographic or video evidence of abatement or other written records.

29 CFR 1910.303(b)(1): Electrical equipment was not free from recognized hazards that were likely to cause death or serious physical harm to employees:    (a) South AC - The roller conveyor line, used to move pipe upriver or down river out of the building, had two electric-powered operator control (switch/button box) stations.  Two employees worked to operate the roller conveyor line, one employee per switch/button box station.  These two electric control stations had deficiencies, as follows.  Either of these switch/button box stations would operate the roller conveyor line, but the switch/button box stations did not incorporate an interlocking feature to prevent the activation of the roller conveyor line except by the concurrent use of both switch/button box stations.  Also, the switch/button box station on the upriver side did not have an emergency stop function, and some of the switches/buttons on both switch/button box stations did not have clear labeling, exposing employees to struck-by and caught between hazards, on or about October 12, 2016.    Abatement certification and documentation required within 10 days after abatement date.
