29 CFR 1926.1416(e)(1): Where crane equipment did not have a boom angle or radius indicator readable from the operator's station, alternative measures were not used to determine the booms angle or radius. Worksite: The Grove telescopic boom crane that was used in pile driving steel I-beams piles was not provided with a operational load moment indicator (LMI)/boom angle indicator. On or about 6/14/2012, the above Grove telescopic boom crane tipped over while perfroming the above operation as a result of being overloaded.
