29 CFR 1926.25(a):     During the course of construction, alteration, or repairs, form and scrap lumber with protruding nails, and all other debris, were not kept cleared from work areas, passageways, and stairs, in and around buildings or other structures.    On or about March 14, 2012, the employer did not ensure all scrap lumber with protruding nails and debris were kept clear from the work area and passageways around the building exposing employees to impalement hazards, slips, trips, and falls.

On or about March 14, 2012, the employer did not ensure each employee, on a walking/working surface with an unprotected side or edge, was protected from a fall hazard exposing employees to a fall of more than 6 feet.
