29 CFR 1926.451(g)(1)(v): Each employee on a walkway located within a scaffold was not protected by a guardrail system (with a minimum 200 pound toprail capacity) installed within 9.5 inches (24.1 cm) of and along at least one side of the walkway. At the job site, on June 18, 2012, employees were working on a second tier of a scaffold that was greater than 10 feet without mid rails or top rails exposing them to falls from heights.
