29 CFR 1926.451(b)(1): Platform(s) on all working levels of scaffolds, were not fully planked or decked between the front uprights and guardrail supports such that the space between adjacent units, and the space between the platform and the uprights is no more than 1 inch wide:    a) On the east side of the building, where a tubular welded scaffolding system had was not fully planked on the fourth and fifth tiers.  Employees were applying stucco from these levels.

29 CFR 1926.451(c)(1)(ii): Guys, ties, and/or braces were not installed  according to the scaffold manufacturer's recommendations or at the closest horizontal member to the 4:1 height and repeated vertically at locations of horizontal members every 20 feet (56.1 m) or less thereafter for scaffolds 3 feet ((0.91 m) wide or less, and every 26 feet (7.9 m) or less therafter for scaffolds greater than 3 feet (0.91m) wide:     a) On the east side of the building, where a 3-foot wide tubular welded frame scaffold was not restrained from tipping at 12 feet and 31 feet.

Crossbraces were used as a means of access:    a) At the worksite, a 3 foot wide , 5 tier high tubular welded scaffolding system was not provided with a safe means of accessing platforms.
