29 CFR 1926.501(b)(13):     Each employee engaged in residential construction activities 6 feet or more above lower levels was not protected by guardrail systems, safety net systems, or personal fall arrest systems, nor did the employer demonstrate that it is infeasible or creates a greater hazard to use these systems and develop and implement a fall protection plan which meets the requirements of paragraph (k) of section 1926.502:      a) Various locations on residential structure: On June 13, 2012 and at times prior thereto, employees performing roofing activities at three different locations on the roof, wore fall protection harnesses but were not attached to the rest of the fall protection systems that were in place.  The roof varied from 2 in 12 to 6 in 12 pitch, and the fall hazard ranged from 10 to 20 feet.     Note) Madison Roofing, Inc., was previously cited for a violation of this Occupational Safety and Health Standard 29 CFR 1926.501(b)(13) that was contained in OSHA inspection #313355448, citation 2, item 1, and was affirmed as a final order on August 31, 2010, with respect to the work site at 3003 Cadet Lane, Boise, ID.       Note) Madison Roofing, Inc., was previously cited for a violation of this Occupational Safety and Health Standard 29 CFR 1926.501(b)(13) or it's equivalent 29 CFR 1926.501(b)(10) that was contained in OSHA inspection #313352866, citation 1, item 1, and was affirmed as a final order on April 7, 2010, with respect to the work site at 3330 West Grace, Boise, ID.     Note) Abatement certification is NOT required for this item.
