29 CFR 1926.501(b)(1):     Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems:    A.)  Javier Lopez, 1213 Peach Street, Erie, Pennsylvania  16501, east side of the structure, on or about June 21, 2012 and time prior thereto:  An employee of the company was standing at edge on the roof of the structure at a height of 36 feet during scaffold erection.  Work activities included but were not limited to tying the scaffold into the building.
