29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems:   a) Job site, 366 Lincoln Boulevard, Pittsburgh, PA: On or about April 30, 2018, during roofing activities, the employer did not ensure that employees used appropriate fall protection systems/devices on roofs determined to have an approximate 9 in 12 pitch where fall hazards of up to approximately 30 feet existed.

29 CFR 1926.102(a)(1): The employer did not ensure that each affected employee used appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation:   a) Job site, 366 Lincoln Boulevard, Pittsburgh, PA: On or about April 30, 2018, during roofing activities, the employer did not ensure that employees used appropriate eye/face protection when operating nail guns.  Javier Lopez, dba J&G Lopez Roofing was previously cited for a violation of this occupational safety and health standard or its equivalent standard, 29 CFR 1926.102(a), which was contained in OSHA inspection number 1279751, citation number 1, item number 1, and was affirmed as a final order on December 18, 2017, with respect to a workplace located at 301-307 Lincoln Avenue, Pittsburgh, PA.

29 CFR 1926.1053(b)(15): Ladders were not inspected by a competent person for visible defects on a periodic basis and after any occurrence that could affect their safe use:   a) Job site, 366 Lincoln Boulevard, Pittsburgh, PA: On or about April 30, 2018, during roofing activities, the employer did not ensure that the extension ladder, used to get up/down from the roof was in safe condition to use, as labeling was either illegible or missing.
