29 CFR 1926.451(g)(1)(i): Each employee on a boatswains' chair, catenary scaffold, float scaffold, needle beam scaffold, or ladder jack scaffold shall be protected by a personal fall arrest system: a) On or about June 15, 2012, at a jobsite located at 13042-52 S. King Drive Blvd., Chicago, IL - Employees were performing aluminum siding activities approximately twelve (12) feet above the ground from a ladder jack scaffold without the use of a personal fall arrest system, thereby exposing the employees fall hazards.

29 CFR 1926.451(e)(1): When scaffold platforms are more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from another scaffold, structure, personnel hoist, or similar surface shall be used. Crossbraces shall not be used as a means of access. a) On or about June 15, 2012, at a jobsite located at 13042-52 S. King Drive Blvd., Chicago, IL - Employees were performing aluminum siding activities approximately twelve (12) feet above the grouns from a ladder jack scaffold without an additional ladder for safe mean of access, thereby exposing the employees fall hazards.
