29 CFR 1926.451(b)(5)(ii): The end of a platform greater than 10 feet in length extended over its support more than 18 inches and the platform was not designed and installed so that the cantilevered portion of the platform is able to support employees without tipping, or has guardrails which block employee access to the cantilevered end: a. On or about March 22, 2017, at the above addressed jobsite, employees were performing fascia and soffit operations from a ladder jack scaffold. The end of the platform extended more than 18 inches over the scaffolding support, thereby exposing the employees to fall hazards of approximately twenty (20) feet, six (6) inches.

29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, structure, personnel hoist, or similar surface was not used. Cross braces were used as a means of access. a) On or about March 22, 2017, at the above addressed jobsite, employees performing fascia and soffit operations from a ladder jack scaffold at approximately twenty (20) feet, six (6) inches were not using an additional ladder for safe means of access, thereby exposing the employees to fall hazards. The BTS Construction, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.451(e)(1), which was contained in OSHA inspection number 482518, citation number 1, item number 2 and was affirmed as a final order on September 7, 2012 with respect to a workplace located at 13048-52 S. King Drive, Chicago, IL 60628.
