29 CFR 1926.501(b)(11): Each employee on a steep roof with an unprotected sides or edges 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems. a) Motel Roof - On or about 26 June, 2012 - During roof installation, employees were installing roof decking on a 5 on 12 pitched roof deck without properly utilizing conventional fall protection, CSHO observed employees working on the roof without safety harnesses connected to a lifeline. Employees were exposed to a fall hazard of 20 feet.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladders length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed: a) Motel Front - On or about 26 June, 2012 - Employees were using a ladder to gain access to the roof that was not extended a minimum of 3 feet above the landing surface. Employees were exposed to a fall hazard of 20 feet.
