29 CFR 1910.134(f)(2): The employer did not ensure that an employee using a tight-fitting facepiece respirator was fit tested prior to initial use of the respirator, whenever a different respirator facepiece (size, style, model or make) is used, and at least annually thereafter (a) On or about 6/22/12 an employee working as the Refrigeration Supervisor, using a tight fitting respirator was not fit tested annually. The employee used a tight fitting air purifying full face respirator to enter, diagnose, and repair an ammonia leak in the plant. The employee was fit tested last on 10/27/2010. The employee was fit tested in 2010 with a Scott respirator, not a 3M. (b) On or about 6/22/12 an employee working as a refrigeration mechanic was not fit tested prior to using a tight fitting respirator. The respirator was a 3M full face respirator with cartridges used to enter, diagnose, and repair an ammonia leak in the plant. Employees are exposed to the hazard of exposure to ammonia which may cause lung damage.
