The employer does not ensure it documented that equipment in the process complies with recognized and generally accepted good engineering practices (RAGAGEP).    a. The violation occurred on or about October 15, 2014 and at times prior thereto in system 1 and system 2 engine rooms where employees were exposed to fire, explosion or inhalation hazards of ammonia when the employer failed to ensure that detectors activate visual and audible alarms outside each entrance to refrigeration machinery rooms in accordance with RAGAGEP such as ANSI/IIAR 2-2008.      b. The violation occurred on or about October 15, 2014 and at times prior thereto in the system 2 engine room where employees were exposed to fire, explosion, or inhalation hazards of ammonia when the employer failed to ensure that the system 2 relief header discharged was to a safe location in accordance with RAGAGEP such as ANSI/IIAR 2-2008 and ANSI/ASHRAE 15-2010.      c.  The violation occurred on or about October 15, 2014 and at times prior thereto in the system 2 engine room where employees were exposed to fire, explosion, or inhalation hazards of ammonia when the employer failed to ensure that the entrance and exit doors were tight-fitting in accordance with RAGAGEP such as ANSI/IIAR 2-2008 and/or ANSI/ASHRAE 15-2010.      d.  The violation occurred on or about October 15, 2014 and at times prior thereto in the system 2 engine room where employees were exposed to fire, explosion, or inhalation hazards of ammonia when the employer failed to ensure that the allowable piping discharge lengths were in accordance with RAGAGEP such as ANSI/ASHRAE 15-2010.      Pursuant to 29 C.F.R. 1903.19, within ten (10) calendar days of the abatement date the employer must submit documentation showing it is in compliance with the standard, including describing the steps it is taking to ensure the equipment in the process is documented that it complies with recognized and generally accepted good engineering practices.
