Cuddy Roofing Company, Inc., 2200 Memorial Road, Farrell, PA 16121, UPMC Horizon-Shenango Valley, on or about June 27, 2012 and times prior thereto: The extension cord which supplied power to a Milwaukee Sawzall (S/N: C24A610200031) had damaged strain relief on the male side of the cord exposing the inner insulated wires.

An access path was not erected from the ladder to the warning line system.
