29 CFR 1926.404(b)(1)(i): Employer did not use either ground fault circuit interrupters as specified in paragraph (b)(1)(ii) of this section, or an assured equipment grounding conductor program as specified in paragraph (b)(1)(iii) of this section to protect employees on construction sites. Behind the Army Nation Guard Main Office on or about July 9, 2012, and at times prior thereto, there was no assured equipment grounding conductor program or ground-fault circuit-interrupter program when a 50 foot extension cord was used to provide power to a Bosh 1810PSD, 4-1/2" Paddle Switch Grinder.
