29 CFR 1910.134(k)(1): The employer did not ensure that each employee can demonstrate knowledge of the items in (i)-(vii) (a) An employee (Abrasive Blaster) that wears a Bullard Model 88VX supplied air helmet while conducting abrasive blasting operations was not provided training on all of the topics listed in paragraphs (k)(i) through (k)(vii) of this section including, but not limited to, how improper care and storage of respirators can compromise the protective effect of the respirator as determined on July 18, 2012. ABATEMENT CERTIFICATION REQUIRED

29 CFR 1910.134(h)(2)(i): Respirators were not stored to protect them from damage, contamination, dust, sunlight, extreme temperatures, excessive moisture, and damaging chemicals or were not packed or stored to prevent deformation of the facepiece and exhalation valve: (a) Abrasive Blasting Room: The two supplied air helmets used by employees for abrasive blasting were stored by hanging them without any protective covering at the edge of the abrasive blasting area as observed on July 13, 2012. ABATEMENT CERTIFICATION REQUIRED.
