OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees Engine room - On or about July 12, 2012, employee(s) were not adequately protected against the potential release of anhydrous ammonia in that: a. An ammonia detector inside the engine room with audible and visual alarms, also on the outside of the doors communicating with it were not provided. b. No provision existed for inlet air to replace the air being exhausted by the engine room's mechanical ventilation system. c. Doors communicating with the engine room were not self-closing d. Emergency shutdown procedures, including precautions to be observed were not posted on a conspicuous card outside the engine room adjacent to each door. e. Eye-wash and shower facilities were not provided for use by employee(s) in case of physical contact with anhydrous ammonia.
