On or about October 23, 2012, in the facility, employees were required to access and work on an open sided platform, the smokehouse, which was 113 inches high.  The portable step ladder used to access the smokehouse did not allow for proper access to the platform for it was only 67 inches tall and was not intended for that use type.  Employees were not provided with fixed stairs to access the elevated work platform thereby exposing employees to hazards associated with falls.
