29 CFR 1910.303(c)(3)(i): All splices and joints and the free ends of conductors were not covered with an insulation equivalent to that of the conductors or with an insulating device identified for the purpose. Outside Crew Rest Container, South Side of Marine Dock area: On or about 11 July 2012 and times prior thereto, an electrical extension cord had been spliced with automotive type slices and was wrapped with black electrician tape. This exposed employees to a 120v electrical shock hazard.
