(a) Maintenance shop, Break room:  On or about 7 October 2014 and times prior thereto, the 208 volt electrical circuit breaker panel was missing a blank.  Opening covered with black electrician tape.  This condition exposed employees to an electrical shock and burn hazard.  (b) Maintenance shop, East wall:  On or about 7 October 2014 and times prior thereto, the 120 volt electrical junction box missing a knock out.  this condition exposed employees to an electrical shock and burn hazard.
