On or about July 19, 2012, and at times prior thereto, over 140 gallons of flammable materials were stored in an unapproved flammable storage container exposing employees to serious injuries from fire hazards. The container was not equipped with a fire protection sprinkler system.

On or about July 19, 2012, and at times prior thereto, personal fall arrest systems were not inspected prior to each use for wear, damage, and other deterioration, and/or defective components were not removed from service exposing workers to serious injury and harm. The lanyards were found to be covered in paint and other worksite debris. The plastic retractable casings were also cracked causing the lanyards to get caught in the casings.
