29 CFR 1926.453(b)(2)(v): A body belt with lanyard attached to the boom or basket was not worn by employee(s) working from an aerial lift: (a) At the project on or about July 18, 2012, and at times prior thereto, an employee operating a Grove AMZ68XT Articulating Boom Aerial Lift, was not attached/tied off to the anchorage device that was integrated into the work platform structure. The employee attached/tied off to the inner rail of the basket, rather than the manufacturer approved anchorage device that is integrated into the work platform structure.

29 CFR 1926.454(a): The employer did not have each employee who performed work while on a scaffold trained by a person qualified in the subject matter to recognize the hazards associated with the type of scaffold being used and to understand the procedures to control or minimize those hazards. (a) At the project site on or about July 18, 2012, and at times prior thereto, an employee utilizing a Grove AMZ68XT Articulating Boom Aerial Lift, did not understand the procedures to control or minimize hazards associated with the lift. Employee utilized a full body harness with a lanyard and attached onto the inner rail of the basket, rather than the manufacturer approved anchorage device that is integrated into the work platform structure.
