29 CFR 1926.300(d)(3): Other hand-held powered tools, such as circular saws, chain saws, and percussion tools without positive accessory holding means, were not equipped with a constant pressure switch that would shut off the power when the pressure is released:    (a) On October 26, 2016, at the back of the house (2nd floor), a portable electrical drill with a concrete mixing paddle, was not equipped with a constant pressure switch.  Employee was exposed to struck-by and electrical hazards.

29 CFR 1926.404(b)(1)(i): Employer did not use either ground fault circuit interrupters as specified in paragraph (b)(1)(ii) of this section, or an assured equipment grounding conductor program as specified in paragraph (b)(1)(iii) of this section to protect employees on construction sites.    (a) On October 26, 2016, at the back of the house (2nd floor), there was no assured equipment grounding conductor program or ground-fault circuit-interrupter protection when employees used a portable electric drill.
