Each employee on a walking/working surface (horizontal and vertical surface) with an unprotected side or edge which is 6 feet (1.8 m) or more above a lower level shall be protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems. (a) Mobil Station, Dededo, GU: On or about July 19, 2012, and at times prior thereto, employees were exposed to fall hazards of approximately 12 feet 7 inches while installing and testing a refrigerator unit without fall protection.

29 CFR 1926.1053(b)(5)(i): Non-self-supporting ladder(s) were not used at an angle such that the horizontal distance from the top support to the foot of the ladder was approximately one-quarter of the working length of the ladder: (a) Mobil Station, Dededo, GU: On or about July 19, 2012, and at times prior thereto, employees were exposed to fall hazards of up to approximately 12 feet 7 inches while installing and testing a refrigerator unit while improperly using a portable extension ladder. The horizontal distance from the top support to the foot of the ladder was erected approximately 1.9 feet in excess of 1/4 of the working length.
