29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.    The employer does not protect employees from falls on walking/working surfaces were not protected from falling through holes more than six feet above lower levels. This violation was observed on or about July 17, 2012, 2nd floor, Mechanical Mezzalen, where employees were exposed to more than a 6 foot fall hazard while working around holes in walls without guardrail systems erected (south and north) of the stairway.

29 CFR 1926.501(b)(4)(i): Each employee on walking/working surfaces was not protected from falling through holes (including skylights), more than six feet (1.8 m) above lower levels, by personal fall arrest systems, covers, or guardrail systems erected around such holes.    The employer does not protect employees from falls on walking/working surfaces were not protected from falling through holes more than six feet above lower levels.  This violation was observed on or about July 17, 2012, 2nd floor,  Mechaniical Mezzaleen, where employees were exposed to more than a 6 foot fall hazard, when accessing the 2nd floor, a hole (east of stairway) in floor less than 2 feet away from walking/working surface.

29 CFR 1926.1052(c)(12):  Unprotected sides and edges of stairway landings were not provided with guardrail systems which met the criteria contained in Subpart M of 29 CFR 1926.    The employer does not protect employees from falls when using a stairway with unguarded unprotected sides and edges of a stairway landing.   This violation was observed on or about July 17, 2012, 2nd floor of a Mechaniical Mezzaleen where employees were exposed to a 12 foot and 6 inch fall while climbing stairway to the 2nd floor to access the roof.
