29 CFR 1926.451(g)(1)(vii):     Each employee on a scaffold, not otherwise specified in paragraphs (g)(1)(i) through (g)(1)(vi) of this section, more than 10 feet (3.1 m) above lower level was not protected from falls by the use of personal fall arrest systems or guardrail systems meeting the requirements of paragraph (g)(4) of this section:    a)  On or about 09/17/12, an employee was exposed to a fall of approximately 10'5" to the concrete ground below while using a, Sky Jack, Model SJ111 3219, sn:  60626, scissor lift and standing on the top rails of that lift.    b)  On or about 09/17/12, an employee was exposed to a fall of approximately 10'5" to the concrete ground below while using a, Sky Jack, Model SJ111 3219, sn:  60626, scissor lift without the midrail access chain attached.    c)  On or about 09/17/12, two employees were exposed to a fall of approximately 22' to the concrete ground below while using a, Sky Jack, Model SJ111 3219, sn:  55861, scissor lift by climbing on the guardrails to access the roof of the building.
