29 CFR 1926.350(a)(9): Compressed gas cylinder(s) were not secured in an upright position. On or about 21 May 2014, and at times prior thereto, oxygen and acetylene cylinders, both empty and full, were not secured in an upright position at the worksite, exposing employees and others working nearby to the hazard of being struck by a flying projectile and to an explosion hazard.

29 CFR 1926.350(a)(10): Oxygen cylinders in storage were not separated from fuel-gas cylinders or combustible materials (especially oil or grease), a minimum distance of 20 feet (6.1 m) or by a noncombustible barrier at least 5 feet (1.5 m) high having a fire-resistance rating of at least one-half hour. On or about 21 May 2014, and at times prior thereto, oxygen and acetylene cylinders were being stored together without any separation, exposing anyone nearby to the hazard of being struck by flying objects from an explosion.
