29 CFR 1910.95(g)(6): At least annually after obtaining the baseline audiogram, the employer did not obtain a new audiogram for each employee exposed at or above an 8-hour time-weighted average of 85 decibels : a.) ILSCO Extrusions, Inc., 93 Werner Road Building A, Greenville, PA 16125, on or about July 16, 2012, Quest Technology NoisePro dosimeters were placed on the Extrusion Press Operator, Dye Repairman, and the Extrusion Specialist (Finish Saw area) for totals of 463, 466, and 465 minutes, respectively. The equivalent 8-hr dose and Time Weighted Average (TWA) for the Extrusion Press Operator was 175.8% and 94.3 dB. The equivalent 8-hr dose and Time Weighted Average (TWA) for the Dye Repairman was 117% and 91.3 dB. The equivalent 8-hr dose and Time Weighted Average (TWA) for the Extrusion Specialist was 61.1 % and 86.6 dB. Company employees had not been provided with annual audiograms in 2011, and 2012.

29 CFR 1910.151(c): Where employees were exposed to injurious corrosive materials, suitable facilities for quick drenching or flushing of the eyes and body were not provided within the work area for immediate emergency use: a.) ILSCO Extrusions, Inc., (Dye shop-water bath area) 93 Werner Road Building A, Greenville, PA 16125, on or about July 16, 2012, and times prior thereto: Company employees work with and around corrosive chemicals including, but not limited to, Sodium Hydroxide. While a plumbed emergency eyewash and shower was available in an area adjacent to the water bath area, the unit was not maintained in compliance with ANSI Z358.1-1998 for Emergency Eyewash and Shower Equipment in that: 1. Weekly inspections of the equipment were not conducted. The emergency eyewash unit was not activated weekly to verify proper operation.
