Column F of the OSHA Form 300 for 2018 describes the injury or illness, and the parts of the body affected, but does not include the object/substance that directly injured or made the person ill. B.) Ilsco Extrusions, Inc., 93 Werner Road, Greenville, PA 16125, on or about May 22, 2018 and times prior thereto: An employee received a smashed fingertip. Column F of the OSHA Form 300 for 2018 describes the injury or illness, and the parts of the body affected, but does not include the object/substance that directly injured or made the person ill.
