29 CFR 1910.134(c):     The employer did not develop and implement a written respiratory protection program with required worksite-specific procedures and elements for required respirator use:    a)  Max 200 Performance Dog Equipment , Inc., on or about 07/19/12:  The employer had not established a written respiratory protection program when employees are required required to wear required to wear half face positive and negative pressure air purifying respirators.  The program was deficient in that it did not include the following; Medical evaluations of employees required to use respirators; Procedures for proper use of respirators in routine and reasonably forseeable emergency situations; Procedures and schedules for cleaning, disinfecting, storing, inspecting, repairing, discarding and otherwise maintaining respirators and procedures for regularly evaluating the effectiveness of the program.    MAX 200 PERFORMANCE DOG EQUIPMENT, INC., was previously cited for a violation of this Occupational Safety and Health standard or its equivalent standard 29 CFR 1910.134(c) which was contained in OSHA inspection number 189959, citation 1, item 1(a), issued on 05/07/12, with a final order date of 06/06/12, with respect to a workplace located at 2113 State Route 31, Port Byron, New York.    Abatement certification is required for this item.
