(a) Chalan Chiba Lot R91 on July 23, 2012 and at times prior thereto the employer failed to provide neither a ground fault circuit interrupter nor an assured equipment grounding program for temporary electric power at the construction site. Employees in the frequently wet location were potentially exposed to electrical shock.
